Other Forms

  1. Copyright Acknowledgement Form

In order to provide educational materials in accessible formats, RFB&D is required by law to have members sign a copyright acknowledgement form.

  2. Student Certification Form in PDF Format

By the end of your school's (or institution's) membership year, before the next year's membership renewal can be processed, a Student Certification Form must be completed and submitted to RFB&D. The form lists each student served along with a signature by a qualified professional in the field of disability services, special education, medicine or psychology. The certifier must be a recognized expert who attests to the physical basis of the visual, perceptual or other physical disability that limits your use of standard print. Learn more about Certification.

  3. Printable Order Forms

These order forms are not required to place an audiobook or player order; however, for your convenience, we have made them available. You also may order audiobooks and players online by clicking the Member Login button above.

Note to individuals living abroad: If you are an individual outside of the U.S., you may register, but you may only borrow books that are in RFB&D's Classic Cassettes format. RFB&D does not distribute digital versions of audiobooks outside of the United States, except to U.S. citizens who are temporarily residing abroad.

Note to Institutions located outside the U.S.: Currently, RFB&D cannot register schools and other institutions that do not have a U.S. address. A U.S. address includes schools that are in the 50 states, U.S. embassies or military addresses in foreign countries, and addresses from U.S. territories.
