Individual Membership Renewal Form
For those students whose school has identified RFB&D’s recorded books as an appropriate accommodation, their school is required to provide access to RFB&D’s recorded books in compliance with federal disability legislation such as the Individuals with Disabilities Education Act (IDEA) and Section 504 of the Rehabilitation Act of 1973. If you would like a school to pay RFB&D’s membership fee on behalf of the member, choose the "Student Individual Membership" option below and submit this completed form to the school for payment. Please note that schools already may be registered as an RFB&D Annual Institutional Member (AIM) through which the member’s needs may be met. A school may decline payment for a Student Individual Membership if it feels its AIM membership offers the student an appropriate accommodation. For more information on the rights of students with disabilities, visit the U.S. Department of Education, Office of Special Education and Rehabilitative Services web site at www.ed.gov/offices/OSERS. You may also call the U.S. Department of Education at 800-872-5327. Alternatively, by selecting the "Personal Individual Membership" option, the member elects to pay membership fees directly to RFB&D. Members may choose this option for a variety of reasons. For example, they may wish to borrow books that are not part of the school’s curriculum; they may not wish to disclose their disability to their school; their school may not recognize their disability; they may not be enrolled as a student; or they may have other personal reasons.
Check one of the renewal options below: NON-RENEWAL. I do not wish to renew my membership at this time. REASON STUDENT INDIVIDUAL MEMBERSHIP (PAID FOR BY THE MEMBER’S SCHOOL) Submit this form to the school for payment. Have the school return this form along with the $35 annual renewal payment to RFB&D. Membership renewal will be processed once payment is received from the school.
PERSONAL INDIVIDUAL MEMBERSHIP (PAID FOR BY THE MEMBER) As previously noted, this option is appropriate if members would like to borrow books that are not part of a school’s curriculum; they do not wish to disclose their disability to their school; their school may not recognize their disability; they are not enrolled as a student; or they may have other personal reasons. Note: If payment of RFB&D’s membership fee is a financial burden, please call RFB&D’s member services department at 1-800-221-4792 to request a Personal Individual Membership fee waiver form.
RFB&D will process the annual membership renewal upon receipt of this completed form and the $35 annual payment.
Please read the statement below and sign at the bottom. Your membership renewal cannot be processed without a signature. The contents of all RFB&D books are protected under copyright law. RFB&D strictly regulates its distribution of materials to a specialized member population that has provided documented evidence of a print disability. Copying, sharing or redistributing RFB&D's books in any form to any person is strictly prohibited by law and is a violation of publishers’ rights and the terms of your membership. Violators face permanent suspension of RFB&D membership benefits and possible civil or criminal penalties.
